Monopolar RF: The Deepest-Penetrating Configuration Behind Thermage-Style Tightening

MEDICAL AESTHETICS · TECHNICAL GLOSSARY

01

What Is Monopolar RF?

A monopolar RF handpiece carries a single electrode: current flows from the tip through skin, subcutaneous fat and even fascia, returning via a neutral electrode (grounding pad) placed elsewhere on the body. Because the current path spans the full tissue thickness, monopolar is the deepest-penetrating RF configuration with a large heating volume.

02

Technical Essentials

  • Frequency: the classic choice is 6.78 MHz, balancing depth and safety.
  • Capacitive coupling: a dielectric membrane between tip and skin spreads current evenly and prevents arcing.
  • Cooling inversion: cryogen cooling protects the epidermis and shifts the heat peak deeper — the key design letting monopolar heat full-thickness dermis and fibrous septa.
  • Grounding pad: must adhere properly or current concentrates and burns.
03

Strengths and Challenges

  • Strengths: deepest action (full dermis, subcutis, in some systems the superficial fascia), high single-session energy, longest clinical track record.
  • Challenges: pronounced pain from deep heating, so energy is titrated with pain feedback; longer sessions; demanding power and cooling requirements.

Relation to Thermage

Thermage is Solta Medical’s monopolar RF device and trademark; the name is often used generically for monopolar tightening. Comparable devices are marketed as “monopolar RF” — judge them by frequency, energy range, cooling method and regulatory clearance. LEIJEX’s vacuum-assisted RF platform takes a related but distinct route; see the product page.

FAQ

Why is monopolar more painful than bipolar?

Current crosses pain-receptor-rich deep dermis and fibrous septa over a broad volume; pain is also the protective signal for excess energy, so proper technique uses it for titration.

What if the grounding pad is misplaced?

Poor contact concentrates current density at the edges and can burn the pad site — hence the pre-treatment pad check and device self-test.
