The SMAS Layer: Where the Foundation of Facial Lifting Actually Sits

01

What Is the SMAS?

The Superficial Musculoaponeurotic System is a fibromuscular layer enveloping the facial expression muscles and linking skin to deep fascia, formally described by Mitz and Peyronie in 1976. It continues superiorly into the temporal fascia and inferiorly into the platysma, anchoring the retaining ligaments — one essence of facial aging is descent of the SMAS and its ligaments.

02

Location and Thickness

In the cheek it sits around 4-4.5 mm deep (individual variation tracks subcutaneous fat) and is 1-2 mm thick. Layer arrangements differ across forehead, temple and midface — hence region-by-region treatment mapping.

03

Why All Lifting Points at the SMAS

  • Surgery: the core of a facelift is SMAS flap dissection, tightening and suspension.
  • Focused ultrasound: the 4.5 mm cartridge is designed for the superficial SMAS in the cheek, contracting it via coagulation points.
  • RF: most energy stops at dermis and septa; monopolar partly reaches deeper, but with less controllability than ultrasound focusing.
04

Variation and Safety

In fuller faces the SMAS lies deeper, in thin faces shallower — fixed-depth protocols can miss. Fat-atrophy complications of focused ultrasound typically occur when high-energy foci land inside fat. Pre-treatment ultrasound measurement or body-type-adjusted cartridges are the mitigations.

FAQ

Does a 4.5 mm cartridge always reach SMAS?

No — it is a design depth. Excess fat puts the focus inside fat, which is why multi-depth cartridge sets (1.5/3.0/4.5 mm) must be chosen per patient.

Will tightening the SMAS stiffen the face?

Non-invasive coagulation points do not cut muscular structures; at proper energy there is no facial-nerve injury or stiffness. Transient stiffness after surgical SMAS work resolves within weeks.
